Waiver of Liability and Photo Release:

 

In consideration for being allowed to participate in any activities or services provided by Pivotal Play Soccer Training, LLC, I/we, Parent or Guardian of Participant ("I/WE"), acknowledge, understand, and agree for myself/ourselves and my/our minor child (myself/ourselves and my/our minor child hereinafter collectively, the ''Participant'') as follows: a. The risks of injury from the activities involved are significant, and include the potential for permanent injury, paralysis, and/or death; b. I/WE KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, for the Participant, even if arising from the negligence of the Releasee or others, and assume full responsibility for the Participant’s participation. c. I/WE agree to comply with all terms and conditions for participation. If I/WE observe any unusual, significant hazard during my/our presence or participation, I/WE will remove the Participant from participation and bring such hazard to the attention of the nearest instructor immediately. d. I/WE, for the Participant, my/our heirs, successors, legal representative, assigns, personal representatives and next of kin, hereby RELEASE, INDEMNIFY, AND HOLD HARMLESS PIVOTAL PLAY SOCCER TRAINING, LLC, it’s officers, officials, coaches, trainers, staff, agents and/or employees, other participants, sponsors, advertisers, contractors, and if applicable, owners and lessors of premises used to conduct the event  (each of the foregoing, a ''RELEASEE''), from any and all claims, suits, demands, losses, and/or damage to person or property - whether arising from the negligence of the Releasees or otherwise. e. As the parent(s) and/or guardian(s) of the Participant, I/WE give Pivotal Play Soccer Training, LLC, its successors or assigns, permission to use pictures and/or videos of the Participant taken while attending Pivotal Play Soccer Training, LLC related classes, practices, events, sessions, games, clinics, and/or camps without limitation, notice or remuneration. f. All matters arising out of this waiver will be governed by the laws of the State of Maryland.
